Provides pharmaceutical care including evaluation of medication orders for appropriateness of drug, dose, frequency and adequacy of monitoring, evaluation of drug interactions and allergies, therapeutic monitoring of drugs and provision of formal pharmacokinetic consults,evaluation of antimicrobial therapy, and provides patient and family medication education as required.
